The charge for the initial visit is $200. Subsequent visits are $160. There is a $30 fee for missed appointments and cancellations made without 24 hours notice. A fee of $10 will be charged for a returned check and a $5 monthly fee will be charged for delinquent accounts. The fee for yearly or complete account statements is $5; urgent requests are $25. Copies of records are $25 for the first 10 pages. Reports to attorneys and other third parties are billed at $160 per hour prorated.
I accept payment by cash, check, Visa or Mastercard. Payment is expected at each visit.
Insurance: I am a provider for BlueCross Blue Shield, Providence Preferred Oregon, Providence Health Plan and Pacificare Behavioral Health, Lifewise and Aetna. Please confirm that I am an in-network provider for your health plan, even if listed above.
I am out of network for Cigna Behavioral Health, MHN and Value Options. You may have benefits to see an out-network provider, but your cost per visit will be much greater than for an in-network provider.
