When you have scheduled your first appointment, I will mail paperwork to you or you may download it from this website. Please complete the paperwork and bring it with you to your first appointment. I will also need to make a photocopy of your insurance card.
You may review a copy of our privacy policies in our office or on the website. You may download a personal copy.
